Short Contract
This contract was for the improvement of the companies Safety Management Systems, the company had grown beyond it current policies and procedures from a small operatation to a mid size company. After a gap analysis had been completed by a prospective client several short falls (26) in the program were identified and thus creating the need for improvement. The program was reviewed and items including those identified in the Gap analyisi were addressed and improved.

Being an Oil Field transport company it was imperitive to realine the SMS for all current and future contracts for the company. Programs created included a revision of the disciplinary policy to reflect an improved system for dealing with delinquent employees in their accountabilities during the performance of the work duties; a wildlife awareness program was also developed as the company had experienced incidents on the Hiway with animals. Training was also identified as the company did not have a formal system to confirm operator competency when operating equipment as repairs had increased substantially. Also identified was the need for a formal program to address operator fatigue, a program was developed as well due to several fatigue related incidents the company had experienced. The high turnover of employees identified the requirement for an employee exit program to identify the high turnover rate experienced also identified by the gap analysis and the trending of the same. Most issues identified by the gap analysis were created or improved upon. The companies incident report form was also in need of improvement so anew format was developed and brought in to sevice to improve reporting capabilities for the company and to clients as required.

This project is the Insulation of two 275, 000 barrel oil holding tanks with Dura Joints one piece insulation product. The project required the guidance of a senior safety advisor to build the contractors project safety management plan, and the corporate safety manual as well as completing the ITP for the company to ensure quality control. Leading, mentoring the company’s and field leadership to develop an active positive safety culture to ensure a safe project.

Installation of 65Km 16in pipeline Duties included but were not limited to writing the of the site specific and environmental orientations as well as issuing safe work and ground disturbance permits to employees and contractors as well as new hire orientations to all workers coming on site. Touring the work areas to ensure company safe work procedures were followed during work execution. Liaison with client safety and environmental representatives to ensure project requirements were followed. Completing formal and informal inspections and of equipment and the work area as well the safety administration on a daily basis for compliance to the clients expectations. Resolving short falls observed during field inspections. Completing equipment administration inspections to a high standard ensuring documentation was in place and correctly completed for operation.

Project Night Senior Safety Advisor Boiler Rebuild at Agrium Nitrogen Operations

This project was brought to my attention as Agrium had several First Aid events during the completion of the project, reporting to the project engineer the duration was of a short time period approximately two and a half weeks. During this period I was brought in to be the client (Agrium) representative, having the contractor safety reps reporting to my office. For the period the project field tours with the contractor safety reps were completed and safety short falls were addressed and corrected. Meetings with the contractor safety reps were held to address issues pertaining to the project through the project twice weekly to assist the contractor reps with the challenges at their level to quick resolution. On completion of the project no incident or injury events where reported and the project was completed with great satisfaction of Agrium and start-up was attained with no incident’s

This project focused on the dismantling of approximately 150, 000 tons of scaffold materials from the construction contract.

During this project at Suncor the Journey to Zero was reached without incidents or injuries by the development of a strong employee safety culture. This contracted position was leading the night shift on the condenser re-weld project and the furnace shut down and rebuild reporting to the night shift superintendent and Suncor HSE leads.

During the course of the furnace rebuild
Meeting with Suncor Senior HSE leads and senior construction management as well as coordinating HSE functions including Confined Space Entry work with other contractors to and including field Inspections of all site areas and plant activities coordinating and leading Incident investigations with subcontractors.
Attending Suncor HSE lead coordinators meeting to discuss current trends in Incidents, group Incident review meetings and coordinating training requirements for field leadership and employees.
Also directly in charge of all HSE administration functions including leading and mentoring field leadership to accountability of field leadership in a successful project. Meeting with, Suncor senior night shift and liaison with regards to all HSE matters including reporting to Suncor management during the course of the project.

During these projects which entailed the cutting and inspection and re-welding of the condenser bends for a total of 1600 units completed without incident, leading Melloy to meet Suncor’s journey to zero achievement.
