Responsible for providing high quality, evidence-based care to patients. Monitoring their long terms conditions and providing the necessary treatment. I also administer medications, provide wound care and assess, plan, implement and evaluate achievable goals. Other roles include:
• Communicating with residents and relatives in a caring manner
• Being the main point of contact for patients a communicating on their behalf
• Working effectively as a team member
• Problem Solving
• Maintaining patient records and care plans
• Admissions and discharges
• Liaising with other multidisciplinary team members e.g. doctors, physiotherapists, occupational therapists, dieticians and community psychiatric nurses.
• Carrying out various tests
• Medicine administration via injection, orally and through Peg tubes
• Maintaining patient nutrition
• Using mechanical aids e.g. hoists and stand-aids
• Handover of any patient changes to staff coming on duty
