Tertiary Hospital
Job Description:
1. Admits patient into the ward
2. Prepares for nursing care procedures by observing medical asepsis and ensuring compliance with the “12 rights” before giving medication
3. Identifies nursing need of patient and provides nursing care and procedures including, but not limited to taking and recording vital signs, inserting intra-venous fluids, administering medication and/or blood transfusion and observing patient’s reaction, observing, signs and symptoms and instituting basic remedial action, performing irrigations, catheterizations and giving enemas, assisting patients on their movements, maintenance of proper alignment of their body through good body mechanics and undertaking / assisting patients in bathing and oral hygiene.
4. Provides assistance to and coordinates with the attending physicians by assisting them when examining patients, preparing patients for and assisting physicians on the conduct of diagnostic and therapeutic procedures, carrying out doctors’ order related to patients care and relaying to them observations about the patient and soliciting instructions on what action to take.
5. Coordinates with other work units to generate supplies, support services, and information needed for the performances of nursing care functions.
6. Undertakes the completion of entries related to medication and treatment into the patients’ chart.
7. Ensures the availability and usability of required equipment and supplies related to nursing care within the ward.
8. Responds to patients’ complaints and provides immediate resolution on areas that are within one’s specification of authority; refers unresolved complaints to relevant officers and employees of the institution.
9. Assists patients and their families in their transactions with other units of the hospital.
10. Provides health education; interprets to the patient and his/her family their roles in ensuring successful therapy and rehabilitation while in the hospital.
11. Discharges patients upon advice/orders of the attending physician by coordinating with the various related work units on the completion and settlement of all accounts, ensuring completions of clearance before actual discharge, removing all contraptions and attachments to equipment before discharging patient and prepares discharge plan and provides home medication instructions and health care advice.
12. Performs post-mortem care by identifying the body, removing all the contraptions and attachments to equipment from the cadaver, informing attending physician and informing and coordinating with other realated work units in relation to the processing of clearance to remove body from the hospital.
13. Monitors reading from cardiac monitor, pulse oximeter, defibrillator and ventilators, and operations of incubator; reports to attending physician observations; extracts blood for newborn screening test (special function).
14. Prepares and operates special equipment like phaco machines, ophtha microscope, laparoscopy machine, endoscopy equipment, fetal monitor, etc (special function).
15. Monitors reading from the EC, EEG, audiometry and perimetry machine; conducts stress tests, 2D-echo (special function).

Secondary Hospital (Hemodialysis, Surgicl Ward, NICU)
Job Description:
1. Primes dialyzer and bloodlines and readies dialysis equipment
2. Records patients' medical information and vital signs
3. Cannulates accesses, cleans and dresses permanent and temporary catheters
4. Demonstrates the ability to remove femoral shealths & administer medications, including thrombolytics and secures blood works
5. Identifies irregular dialysis reactions and notifies appropriate medical team members
6. Consults and coordinates with health care team members to assess, plan, implement and evaluate patient care plans
7. Prepares and administers (orally, subcutaneously, through an IV) and records prescribed medications.
8. Monitors and adjusts specialized equipment used on patients, and interprets and records electronic display and initiates corrective action whenever the patient displays adverse symptomatology
9. Provides bedside care for a wide variety of medical patients, including pre- and post-surgery patients
10. Initiates patient education plan, as prescribed by physician. Teaches patients and significant others how to manage their illness/injury, by explaining: post-treatment home care needs
11. Changes dressings, inserts catheters, starts IVs
12. Cares for infants transitioning to extra uterine life
13. Provides age and culturally appropriate care to patient
14. Provides nutritional assessment & specialized feedings
15. Monitors and adjusts specialized equipment used on patients, such as incubators and ventilators
16. Assists treating physician during examination, treatment and procedures, which can include life-saving situations
17. Records all care information concisely, accurately and completely, in a timely manner, in the appropriate format and on the appropriate forms
18. Performs other position-related duties as assigned, depending on assignment setting

Secondary Hospital (ER, OR, Ward)
Job Description:
a. Collects current symptoms, as well as a detailed patient history and then consults and coordinates with health care team members to assess, plan, implement and evaluate patient care plans
b. Performs triage, while considering both physical and psychosocial elements
c. Manages basic life support needs and stabilizes patients until the attending physician is available, based upon nursing standards and protocol
d. Works directly under physicians, assisting them during exams, diagnostic testing and treatments
e. Prepares and administers (orally, subcutaneously, through an IV) and records prescribed medications.
f. Maintains proper supplies and appropriate medical equipment to care for patients
g. Conducts pre- and post-operative education
h. Serves as the communication liaison to family and members of the operating team
i. Ensures that operating on correct patient and the correct procedure is being performed
j. Preps operating room for patient/procedure
k. Monitors patients' physical and emotional well-being throughout the care continuum
l. Reports adverse reactions to medications or treatments in accordance with the policy regarding the administration of medications by a licensed registered nurse
m. Records patients' medical information and vital signs
