● Responsible for the reviewing of prescriptions for appropriateness of therapy to determine ingredients needed and to ensure correct dosage.
● Responsible for inventories, orders/maintains supply of drugs, chemicals & other pharmaceutical stock.
● Responsible for the advising patients and care providers of potential drug interactions, possible side effects, storage information, and instructions to use medications.
● Responsible for the performance of packaging, labeling, measuring, compounding, storing, charging, and recording medications used in the patient care.
● Responsible for Dispensing of medicines following the prescriptions issued by authorized medical practitioner.
● Responsible for the preparation of unit dose and IV admixture in aseptic condition & standard protocols.
● Responsible for compounding and dispensing, assessing & evaluating the pharmacological & non-pharmacological needs of patients & apply the principles of pharmaceutical care, providing pharmacotherapy consultation services, patient counseling, drug information services, drug interactions monitoring dose adjustment etc for providing appropriate clinical services to patients.
● Provide accurate, adequate and timely drug information to professional staff, patients & their families.
● Monitor medication orders & review patient profile to ensure appropriate drug selection & dosage form.
● Lectures to practitioners, physicians, nursing staff & assistant pharmacists on pharmacy & therapeutics.
● Reporting to Head of pharmacy services.
