Job Descriptions
a. Conducts an individualized patient assessment, prioritizing the data collection based on the adult or elderly patient’s immediate condition or needs within timeframe specified by client facility’s policies, procedures or protocols.
b. Conducts ongoing assessments as determined by the adult or elderly patient’s condition and/or the client facility’s policies, procedures or protocols and reprioritizes care accordingly.
c. Develops plan of care that is individualized for the adult or elderly patient reflecting collaboration with other members of the healthcare team.
d. Performs appropriate treatments as ordered by physician in an accurate and timely manner.
e. Performs therapeutic nursing interventions as established by individualized plan of care for the adult or elderly patient and his/her family.
f. Provides individualized patient/family education customized to the adult or elderly patient and his/her family.
g. Documents patient assessment findings, physical/psychosocial responses to nursing intervention and progress towards problem resolution.
h. Initiates emergency resuscitative measures according to adult resuscitation protocols.
i. Maintains confidentiality in matters related to patient, family and client facility staff.
j. Provides care in a non-judgmental, non-discriminatory manner that is sensitive to the adult or elderly patient’s and family’s diversity, preserving their autonomy, dignity and rights.
k. Reports patient condition to appropriate personnel during each shift.
l. Maintains current competency in adult medical surgical nursing.
1. Gathers informations such as patient's name, complaint and history upon arrival.
2. Takes and records precisely patient vital signs, height and weight.
3. Assess nursing needs of the patient.
4. Assist physician when examining the patient.
5. Prepares instrument, assist physician as needed.
6. Gives health teachings.
7. Observes aseptic technique, cleanliness and orderliness of the clinic at all times.
8. Documents all care and treatment given to the patient for future reference.
Emergency Room, Medical/Surgical Ward
1. Gives direct nursing care.
2. Identifies nursing needs of the patients.
3. Plan, give and evaluate nursing patients.
4. Admit and discharged patients.
5. Perform such nursing activities as bathing and oral hygiene.
6. Assist patient in moving, maintaining proper alignment of patient’s body and utilizing good body mechanics.
7. Take and record vital signs.
8. Observes medical sepsis.
9. Observes signs and symptoms, institutes remedial action when appropriate and record these in the chart.
10. Reports observation to the physician.
11. Acts as liaison between the patient’s emotional and spiritual needs.
12. Interprets to the patients and their roles in promoting successful therapy and rehabilitation.
13. Prepare patients for, and assists physician with diagnostic and therapeutic procedures.
14. Carries out doctor’s order.
15. Assists the physician when examining the patients.
16. Performs irrigation and catheterization, gives enemas.
17. Administer and chart medications and the patient’s reaction to medication.
18. Performs post mortem care.
Emergency Room, Medical/Surgical Ward
1. Identifies nursing needs of the patients.
2. Takes vital signs, do nursing procedure such as urinary catheterization, wound dressing, nebulization, gavage feeding, intravenous infusion and suctioning of secretions.
3. Assists physicians when examining the patients.
4. Prepares instruments, assists physicians during minor operation.
5. Admits and discharge patients.
6. Chart medications and treatments given with accuracy.
7. Gives health teachings.
8. Complies with hospital and standards operating procedures.
9. Maintains good relationship with hospital personnel.
10. Observes cleanliness and orderliness in the patient’s unit at all times.
11. Performs post- mortem care.
