Conduct post-service and appropriateness of treatment setting reviews to ensure compliance with applicable criteria, member eligibility, benefits and contracts.
-Ensures member access to medically necessary, quality healthcare in a cost effective setting according to contract.
-Consult with clinical reviewers and/or medical directors to ensure medically appropriate, high quality, cost effective care throughout the medical management process.
Provides and coordinates individualized care and resources to patients, follow medical instructions carefully and consistently as per discharge instructions.
-Manage any unanticipated events or unstable situations and make referrals to supervisor as necessary.
-Administer the Home Care Program in order to ensure that the program is delivered in an appropriate, caring and respectful manner.
-Ensure that care is provided according to all relevant policies, procedures and regulations.
-Consult with family members and other supports to ensure that care is on-going and that all client needs are identified and met.
Keeping and filing of patient’s medical records
-Taking vital signs and anthropometric measurements and assessing patient’s condition and proper disposition
-Ordering and inventory of vaccines and other medicines
-Administration of first aid management as case warrants and facilitating proper referrals to hospital for those in need of admission
-Educating in disease prevention and case management
Monitors vital signs, checks intake and output
• Administers medications, treatments, nursing procedures as prescribed (irrigation, cathetherizations, oxygen therapies, feedings, etc.)
• Assists with other special or complicated procedures (blood extractions, ET intubation, minor dressing, minor incision and drainage, minor and major surgeries etc.)
• Carries out Physician’s orders accurately with good judgment
• Provide therapeutic environment for the patient
• Facilitates admission, transfer and discharge of patients following hospital procedures
• Maintains complete, neat records of patient per shift and whole hospitalization
• Bathes/ sponges patient
• Ensure safety and comfort to patients
• Reports pertinent laboratory results and findings to the medical team through proper channel
• Makes proper referral to Unit/Department/ Agency pertaining patient care and or management or treatment
• Ordering and inventory of vaccines and other clinic equipments
• Educating disease prevention and case management
