Assisted individual patients with elements of basic nursing care and the implementation of programmes of care under the direction of a registered nurse. Contributed to the ongoing support of patients and their relatives. Contributed to the support of patients in transition from one care setting to another. Assisted with obtaining, transmitting and storing information related to the delivery of a care service. Assisted and contributed to the movement and treatment of patients to achieve physical comfort and minimise the adverse effects of pressure according to the prescription of care. Reported areas of concern to qualified staff. Assisted individual patients with activities of daily living according to individual needs including personal hygiene, appearance, oral care, fluid and dietary intake. Developed an understanding of individual patient needs in relation to dietary regimes and fluid intake. Monitored and recorded fluid balance and dietary intake. Reported areas of concern. Enabled/assisted patients to minimise discomfort and pain according to the prescription of care. Participated in clerical duties as required. Supported qualified staff by carrying out accurate record keeping, taking and relaying verbal and written messages promptly, respecting confidentiality. Contributed to minimising the effect of disruptive, abusive behaviour and the protection of individuals from abuse, complying at all times for Health & Safety/Risk Management detailed below. Contributed to the clinical governance agenda. Escorted patients, whose clinical condition has been deemed to be stable by a registered nurse or health care practitioner (e.g. doctor), to and from the Radiology Department, wards and departments. Undertake the collection of routine specimens, e.g. sputum, stool, urine. Undertake physiological observations, e.g. BP, respiration rate, temperature, pulse rate. Report areas of concern. Undertake routine ward / department urinalysis. Undertake routine procedures, which require on the job training, through a local training session, or a combination of instruction and practice for a particular task, reporting any concerns to the registered nurse, but there is no ongoing requirement for monitoring or interpretation of results. Participated other duties as required and appropriate to the level of this post.
