Providing home care physical therapy to patients referred by physician due to recent illness or worsening of a condition. Providing initial and ongoing assessment to determine level of functioning, including OASIS Assessment at appropriate time points, gathers background data, reviews referrals, selects and modifies evaluation tools, and evaluates patients determining the level of patient’s physical and psychosocial functioning. Educate patients and/or caregivers/families about proper safe use and recommend assistive device and/or installation of adaptive equipment for safety in home environment.
