Assesses initially the patient's condition and family to determine care needs and identify a complete physical assessment and history of current and previous illness.
Develops a care plan that establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative long term nursing actions and includes patient and family in the planning process

Performs accurate and complete admission assessments
Assisting doctors with procedures done in the out-patient department
Monitors and documents the patient’s condition and the effectiveness of care and/or interventions
Establishes and implements a care plan based on sound nursing theory, scientific principles, professional judgment and individual patient needs
Evaluates and updates the patient’s plan of care to facilitate the achievement of planned and expected outcomes

Duties and Responsibilities:
* Utilizes assessment skills in determining patient's needs
* Analyzes findings, records and reports assessments
* Gives health instruction and teachings to patient
* Supervises members of the health team in the delivery of quality care
* Implements physician's orders properly
* Performs other responsibilities that maybe delegated by authorities readily and being punctual
* Takes supervision of all staff in the absence of Head nurse
* Participates in the continuing education, research and quality assurance program
