Main Responsibilities:
Assume Primarily Responsibility for total census during assigned shift Answer all phone calls received on behalf of the agency and follow up according to agency-to-agency policies and procedure. Provide skilled nursing intervention and supportive care in patients I home settings. Maintain regular communication with agency staff and the patients referring physicians and or case manager concerning patients or family care needs and response to intervention. Clients:
