Assessment and implementation of planned intervention

provide nursing care to patient at the convenient of there home, following the 4 nursing process, provide safety and document all the care done to patient.

Staff nurse
- takes care of patient from admission till discharge
- responsible for providing quality of care from simple to complex cases.
-provide pleasant & safety enviroment to aid in the healing process of the patient.
- Keeps record about the patients condition and its intervention.

NURSE EDUCATOR:

- Conduct orientation to new staff
- monitor the progress of the new staff in the unit
- conduct assessment of knowledge and skills.
- facilitate probationary evaluation and annual evaluation
- assist nursing quality on the implementation of policies and procedure.

1. Help with the assessment of patient under her care and the newly admitted so as to collect
Relevant date information.
2. Plan the care to be provided by making use of available resources.
3. Implements care that has been organized in an effective care.
4. Evaluates care given and take corrective action.
5. Provides physical, psychological, social, and spiritual needs of patient.
6. Protect the patient from infection and accidents.
7. Prepares the patient and the necessary equipment for diagnostics procedures ans assists the
Physician.
8. Carries out therapeutic procedures delegated by the head nurse and monitor patient’s
Condition.
9. Receives and gives report to patient’s condition and treatment prescribed by the physician to
next shift.
10. Collects samples as requested by the medical team for laboratory investigations.
11. Monitor vital signs and inform the attending physician of any changes in patient’s condition

12. Administers medication as prescribe by the doctor observing the 7 rights of drug
Administration.
13. Admits and discharge patients.
14. Check all equipments on regular basis making certain that they are in good working
Condition, keeps an accurate record of equipment/instruments used in the ward.
15. Maintains a good rapport and professional relationship with medical team and colleagues.
16. Render health teaching to patient and patient’s relatives
17. Reports to the Head Nurse of any immediate problems and difficulties that may arise
during the course of duty.
18. Maintains confidentiality of patient’s records, and updates all relevant charts.
19. Promotes professional development in terms of education, skills, and attitude so as to
maintain current trends and practices.
20. Demonstrate an awareness of fire hazards and the necessary precautions to be taken in the
event of fire.
21. Demonstrate an awareness of the socio-cultural factors involving in nursing practice.
