• Makes home visits to home health services patients performing nursing care based on a complete and thorough assessment thereby formulating a plan of care, nursing interventions of the client’s needs at the same time documenting appropriate observations made and reporting to Physician and Head Nurse.
• Initiates a plan of care and intervention based on patient’s need.
• Demonstrate the ability to observe, record/report needs, concerns and coordinate the needs of home care patient and maintain a constructive relationship with patient and other health team member.
• Supervises the needs of the patient in coordination with other responsible person in meeting nursing and related caring needs of patient.
• Responsible for establishing and maintaining a healthy interpersonal relationship with all the staff and immediate family.
