Manages and provide nursing care activities in home care setting.
Duties and Responsibilities:
•Provide complete health assessment.
•Educate patient and families regarding the importance of health promotion and disease prevention.
•Proficient in working with ventilator patient, hospices care, tracheostomy tube, suctioning, gastrostomy tube, catheters, wound care.
•Performed health assessments on individuals by evaluating their physical, mental and environmental health status; provided nursing intervention based on assessment of needs.
•Evaluate patients and developed appropriate care plans.
•Apply care plans according to individual patients.
•Assess outcomes and managed care plan modifications as needed.
•Administer medication as instructed.
•Manage emergency procedures.
Emergency Room Department
Manages and do direct nursing care activities to patient. Coordinates care planning with other health care team.
Duties and responsibilities:
•Assess the needs of the patients in the ward and make nursing care plan for all patients.
•Delivers direct patient care; assesses patients’ physical, psychosocial and/or emotional need; provides patient and family teaching; triages patient and inform physician of patient status as needed.
•Provide comfort and maintain safety of the patient.
•Ensures efficient and coordinated hospital flow by coordinating and planning activities; communicates patient’s needs and make referrals.
•Administer medications as directed and provide treatment as prescribed by the physicians, cognizant of reactions and report harmful side effect.
•Maintain a good communication channels with health care team and report any occurrence to the senior nurse.
•Participates in health education to prepare the patient and family for discharge. And provide correct information to the patient regarding treatment and follow-up appointments.
