• Fundamental , desirable , optimal duties • Member of Advanced and basic life support team (code blue team ) • Evaluate all drug therapy for appropriate indications, dosage, drug interactions, and drug allergies; monitors the patient’s pharmacotherapeutic regimen for effectiveness and ADEs; and intervenes as needed. • Initiate and/or evaluate orders for parenteral nutrition and recommends modifications as indicated to optimize the nutritional regimen. • Identify ADEs including allergy and assists in their management and prevention, and develops process improvements to reduce drug errors and preventable ADEs. • Use the medical record and communicate with other health care professionals and document specific pharmacotherapeutic recommendations. • Provide pharmacokinetic/dynamic monitoring when a targeted drug is prescribed (Peak, trough, Renal,CRR,IHD, Hepatic and other Machines or medical conditions could alter drug Pharmacokinetics or dynamics) • Provide drug/poison information and intravenous compatibility information to the ICU team. • Provide drug therapy related education to ICU/Pharmacy team members. • Reporting ADEs to institutional committees. • Document clinical activities in progress notes, direct referral form and multidisciplinary rounds` forms (Geriatrics, ICU, CCU and Nephrology) • Educate health professionals regarding current drug-related procedures, policies, guidelines, and pathways. • Initiate drug information newsletters. • Implement and maintain departmental policies and procedures related to safe and effective use of drugs in the ICU. • Provide consultation to hospital committees in critical care pharmacotherapy. • Daily make rounds as a member of the multidisciplinary critical care team. • Review a patient’s drug history to determine which maintenance drugs should be continued during the ICU stay. • Clarify previously effective dosages and dosage regimens. • Assess the patient drug history for causality and documents in the medical record any findings that will impact patient management. • Provide formal nutrition consultation on request. • Provide lectures to health professionals, students in critical care pharmacology and therapeutics. • Participate in training pharmacy students and residents through critical care rotations. • Assist physicians in discussions
