Job Description:
• Receives referrals (Patients) from Physicians of various hospitals and implement Doctor's/Physiatrist's prescribed Physical Therapy treatment.
• Performs assessment, evaluation and reexamination of patients to confirm referring Physician's diagnosis and to determine if prescribed plan of treatment is appropriate.
• Prescribes Physical Therapy treatment and implements interventions as well as modifies plan of care or discontinues the plan accordingly.
• Refers patients to seek consultation from physicians or those who have special skills, knowledge or expertise whenever their welfare will be safeguarded or advanced.
• Educates, give home instructions to patients and family members and provides home care services when advised by referring physician.
