As a Staff Nurse:
Identification of Homecare needs of the patient.
Continuing supervision of care and determining the medical necessity of that care.
Management of nursing problems as per request.
Communication with the patient and other team members and with the physician.
As a Team/Area Leader:
Communicates with the patient, other team members and with the physician
Accountability for staff members’ action and follow-up and assigning of duties for nursing staff under my supervision.
Overseeing admission and charges.
Coordinating with Patient Care Coordinator for matter relating to patient.
Putting into practice company policies and procedures and decisions by the management.
Works as a home Dialysis Nurse:
Assisting patients living with kidney failure through home hemodialysis treatment.
Ensure patient safety while undergoing dialysis and communicating with the physician of the patient’s treatment status.
Start, monitor and perform treatment and continuously keep a close watch on the equipment readings for patients.
Monitor the balance of fluid for patient as well as educate and care for them daily.
Work with patients and teach them how to manage their illness outside of the hospital. This includes dietary needs, nutritional adjustments, exercise plan, and the administration of medication if required.
