Teaching medical students, mainly year 5 (final year) and mainly clinical bedside teaching at the same time when I am free from teaching I go to PICU of Hospital Kuala Lumpur which is the place of teaching my students. I do my clinical attachment in PICU, Hospital Kuala Lumpur as an observer and using ultrasound probe and doing physical examination but I am not writing in patients notes and I am not doing procedures because to do that I must leave MAHSA university to keep full time in the hospital which is difficult for me because MAHSA university is the source of my income and visa.

Active training in PICU hands on work with doing on call duties around 3 times per month as a specialist on call. It is very interesting but I could not continue because it must be full time and unpaid. I get only on call claim. I left mainly because of financial issues and visa issues because the hospital does not provide visa for dependents of those who are in clinical attachment of fellowship
The training period was very interesting to me
The unit has good facilities of conventional mechanical ventilation as well as HFOV and iNO but there is no ECMO.
Non invasive mechanical ventilation also is available
The unit has 10 beds, 2 of them are isolation beds and the admission has rules as well as referral to the unit so the number of the beds in the unit will never increase
I got good experience even before I come to this part of training because I built up my experience in Misrata central hospital especially care of patients on conventional ventilation but I added more experience in this unit by exposure to HFOV and iNO which are not new for me because I exposed to them when I was doing my clinical master program in University of Malaya.
Regarding procedures I do well central venous lines, intubation and chest tubes with the other common procedures in PICU
I also learned how to use the ultrasound machine for heart and brain imaging but I am weak in this area (Radiological evaluation), I can detect pleural effusion, hydrocephalus, pericardial effusion and big abnormalities but I still dont have the full skills of radiological imaging which I am trying to improve it now in my clinical attachment in PICU at Hospital Kuala Lumpur while I am teaching medical students of MAHSA university

I established this unit in Misrata Central hospital and I was appointed as the head of the unit.
The unit has 6 beds but we accept more than that with extra beds because of the demand of the area.
Highest ventilatory support available in the unit is conventional mechanical ventilation. There is no HFOV or iNO or ECMO
Our PICU receives cases of general paediatrics and post operative cases and cases with traumatic injuries with shared care with Neurosurgeons, orthopedic and surgical team.
We dont have post- op cardiac cases and we need to refer the complicated cases after initial resuscitation.

During these postings major work fields are:
-Daily round in the wards as registrar with supervising genior medical officers with initiating the plan of all patients and then discussing major issues with the ward lecturer and consultant
-Performing major procedures like umbilical artery catheterization and peripheral insertion of long lines and supervising genior medical officers with top guiding by the ward lecturer or consultant
-running follow up clinics (BMT clinic, hematology and oncology clinic, post-natal clinic, neonatal clinic, developmental clinic and community clinic)

-Performing 24 hours calls as registrar with hospital stay (4 per month) and starting management of newly admitted patients and reviewing patients who have been passed over. During every call there is a major round supervised by the lecturer on call to discuss major issues
The academic programs are:
- Department conference weekly every Monday afternoon with 2 long cases and one basic sciences presentation presented by pediatric medical officers with 6 short cases from the cases which are managed in University of Malaya Medical Centre. This conference is attended by medical officers, lecturers and consultants and interesting issues are discussed in this conference
- Radiology conference every Tuesday morning with discussion of the major issues considered with radiological findings
- Hospital conference every Wednesday morning with presentation done by lecturers or consultants from different departments of University of Malaya
- Weekly tutorial of a major subject every Thursday morning.
- Journal club or conference report every Friday morning with presentation by lecturers and consultants or registrars with attendance of medical officers and discussion

- 4 intensive courses each is one week duration covering major fields of clinical pediatrics and pediatrics basic sciences
