Supporting a lady with learning disabilities to access the local community.
Newmarket Open Door is a project for homeless youngsters aged 16-24. Duties include completing support plans, attending external appointments with residents, life skills and participating in various activities eg: gym, swimming and exercise classes with residents.
Supporting a lady in a wheelchair to live an independent life with her 2 young children, doing daily chores, cooking, taking the children to the park and doing school runs.
Tayfen house is a resettlement project for homeless people. Duties include advising them about benefits to claim, speaking to external agencies, welfare checking residents if there is cause for concern, monitoring CCTV, booking new people into the project and supporting residents through every step of our housing services. Supporting residents with any concerns they may have, assisting them to gain access to various external agencies and updating SPA, Suffolk Coordination Services and Locata. Also making referrals for safeguarding whenever necessary.
Supporting children with social inclusion to access the many different activity clubs available to them in the area they live. Spending quality time with the youngsters in the hope they will be able to carry on with the activities with their parents and siblings. Ensuring safeguarding principles are upheld.
Supporting 2 adults with learning disabilities access the community, socialise with friends, attend college and assist with their day to day living.
Supporting adults to live independently in their own homes and have full access to the community and assisting with daily living. Also working in a care home environment, helping the elderly with daily living, personal care, cooking, cleaning and laundry
