Duties and Responsibilities
Promotes a culture which facilitates team building, innovation, creativity and cultural sensitivity and enhances the growth and development of the Case Management program including supervision, coaching, mentoring and monitoring to ensure that quality is maintained and learning opportunities are provided

Ensure Case Management staff “partner” with physicians to assist to link the patient with financial, clinical, psychosocial and community services to ensure appropriate transition of care planning and utilization of resources.
Monitors and evaluates appropriate utilization of staff in to ensure that skill mix is appropriate for patient population
Collaborates with personnel from all department/disciplines regarding issues that may negatively impact patient progress along continuum of care, plan for transition and discharge and facilitate solutions as needed
Participates in committees, task forces that are interdisciplinary or interdepartmental; may be required to chair, co-chair or lead such groups.
Acts as a change agent for the organization.
Facilitates a positive working environment across the organization.
Provides input into Executive level decisions.
Supports and works in partnership with senior leaders and physician chairs in setting the strategic direction for the department and input into practices which impact the patient experience.
Monitors and reviews staff skill mix and in accordance with patient mix and uses available information to align staffing and workload.
Builds relationships with staff, peers and colleagues to support interdisciplinary and interdepartmental planning, aimed at improving patient outcomes.
Promotes continuity of care that is patient centered, outcome oriented and focused towards appropriate utilization of resources and transition of care planning.
Ensures that quality standards of staff documentation are maintained according to organizational policy.
Perform rounds to assess, identify problems, determine solutions and guide teams in clinical and operational management of service.
Gather, interpret and utilize data to identify trends and demonstrate cost effective solutions.
Monitors and supports the organizational Key Performance Indicators and Clinical Indicators.
Monitors compliance with International and HA-AD Patient Safety Goals.
Reports on department activity using agreed metrics, opportunity and productivity targets
