• Assess patient nutritional status according to established standards of care and practice guidelines
• Develop nutritional care plan for patients of any age, based on their cultural orientation
• Evaluate the results of the nutritional intervention on the patient nutritional status to determine the need to revise the nutritional care plan
• Implement nutritional care plan, and apply appropriate nutritional interventions to improve patient nutritional status
• Consult with physicians on changes in the nutritional management of patients as required
• Plan menus and other feeding modalities (enteral, parenteral), follow-up on patient's tolerance, and make necessary changes as part of implementing the care plan
• Carry out calorie count as needed
• Follow-up on patient's progress and make necessary adjustments to meet patient needs
• Provide nutritional counseling to patient/care giver to achieve desired changes in eating habits, for inpatients and outpatients
• Tailor the education to meet patient's individual nutritional needs and level of comprehension
• Ensure comprehension, encourage compliance and organize for follow-up sessions as needed
• Document all nutritional assessments, interventions, education and follow-ups in the patient medical record
• Complete the outpatient assessment record for every outpatient consultation
• Participate in community events as part of the multidisciplinary team to perform assessments & interventions including diet education when applicable.
