Create, Implement and maintain site specific LOTO procedures, ensuring compliance with OSHA, and
Company standards and regulations.
• Ensure LOTO procedure is followed correctly
• Work with engineers, supervisors and contractor to plan safe isolation of energy sources (Electrical
Mechanical etc)
• Develop issue and mange LOTO, Isolation permits, Energize & De-Energize permits, maintain logs, control
board and ensuring all documentation is accurate and accessible.
• Train an Affected Person and Authorize Employees before start the activity
• Conduct training sessions and toolbox talk for all personal, including new hire and subcontractor on LOTO
Protocol.
• Identify all energy sources boundary related to the equipment (Electrical & Mechanical)
• Shutdown the Equipment properly
• Isolate and Lock all Energy sources related to requested Active permits and Highlighted Insulation points
• Verify Zero Energy before start Maintenance Activity ( No Power, No Pressure)
• Conduct audits and inspections of LOTO activities.
• Verify all locks and tags are removed safely after work completion.
• Maintain and records all LOTO related activities, Including issuance and removal of Lock & Tags accurate
logs and report weekly.
• Ensure the equipment or systems are in safe configuration after work is completed.
• Maintain list of Authorized permit requestor and only accept requisition form authorized personnel.
