Job description:
•Administer general nursing care to patients in the unit.
a. Performs dialysis procedure.
b. Does pre-dialysis physical assessment.
c. Performs proper cannulation and observe aseptic technique during the procedure.
d. Carries out dialysis prescription, checks good patency of the access, extracts pre and
post dialysis blood specimen for laboratory examination as ordered.
e. Monitors and observes patients for possible complication during the whole treatment.
f. Reminds patient about care and schedule of next dialysis treatment.
g. Carries out OPD, referral and transfer procedures.
h. Records and reports condition and progress of patients under her care.
i. Prepares and administers medication and other treatments as indicated.
j. Gives health teachings to patients and relatives.
k. Ensure the safety of patients during the procedure.
l. Reports to immediate superior/physician for further medical management on complications manifested by patients.
m. Records pre-intra and post dialysis progress and condition of the patients under his/her care.
n. Provides health teaching, with focus on access site care to patients and significant others.
•Gives proper endorsement to the incoming shift.
•Does necessary referrals to other departments concerning patient’s needs.
•Maintains confidentiality of records and other information regarding patient’s care.
•Provides clean, safe, and therapeutic environment.
•Implements hospital rules and regulation.
•Prepare bills, charge slip for hospital supplies and drugs used by patients.
•Responsible in the maintenance and safekeeping of equipment and supplies.
•Attends meeting, conferences and other staff development program.

Job description:
•Primes dialyzer and bloodlines.
•Records patients’ medical information and vital signs.
•Readies dialysis equipment.
•Oversees the dialysis treatment from start to finish.
•Cannulates accesses.
•Administers medications.
•Cleans and dresses permanent and temporary catheters.
•Monitors patient and adjusts specialized equipment used on patients, and interprets and
records information.
•Identifies irregular dialysis reactions and notifies appropriate medical team members
•Secures blood works.
•Teaches patients and significant others how to manage their illness, by explaining: post
treatment home care needs, diet/nutrition/exercise programs, self-administration of
medication, if needed.
•Maintains medical inventory.
•Provides pre- and post-procedure care to patients within the Hemodialysis Unit.
•Performs other position-related duties as assigned, depending on assignment setting.

Performs direct nursing care procedures, which includes bedside nursing, hygiene, pre and post-operative nursing care, as well as admission and discharge duties.
•Administers medication.
•Observes, records and reports patients symptoms, conditions, and progress of treatment and therapy.
•Prepares patients for and assists physicians with diagnostic and therapeutic procedures for both normal and specialized treatment cases.
•Operates diagnostic machines.
•Maintains records reflecting patient condition, medication and treatment.
•Coordinates with other sections in the hospital regarding patient care.
•Assist in the education and rehabilitation of patients and their families in relation to physical and mental health.
•Sets up treatment trays, prepares instruments and equipment, and ensures adequate medical or hospital supplies and availability of proper equipment and instrument.
•Performs other related duties as may be assisgned.
