• Assign codes to diagnosis and procedures using ICD and CPT codes from 3M Coding Encoder.
• Ensure codes are accurate and sequenced correctly in accordance with government and insurance regulations. Communicate with other clinical staff regarding documentation.
• Search for information in cases where the coding is complex or unusual.
• Training the doctors on documentation, Covered and Non-covered diagnosis based on insurance.

Job Profile
•Review medical records and determine if they are complete and accurate.
•Ascertain that medical records are in support of individual risk adjustment score accuracy.
•Develop plans and materials to support education and system changes, to meet both practice and revenue goals.
•Ensure knowledge on deductibles, co-payments, co-insurance amounts, insurance exclusions and other policies of all insurances that hospital is dealing with.
•Educating physicians in Improving Outpatient documentation for reaching correct diagnosis & procedure codes for better reimbursement.
•Review medication list to verify if there is a correlating condition and if conditions are still being treated.
•Review all specialist and hospital consult and lab and pathology reports for new and more specific conditions.
Team Coordinator
•Provide training to staffs periodically.
•Managed executive calendar and coordinated weekly project team meetings.
•Analyse the client feedback to identify and understand the client’s need and offer quality reports.
