Carries out doctor's order and makes sure it's done. Patient is instructed on their next appointment. Coordinates with other wards and makes referral if patients need admission.
As hemodialysis unit head nurse, I provide direction and supervision of administrative and clinical activities in the hemodialysis unit.
As renal transplant coordinator, I prepare potential donors and recipients of kidney transplantation for a safe and successful transplant.
As transplant clinic nurse, I assess patient's condition before doctor's examination and assist them for whatever needs they have.
Make sure that patients are well taken cared of and give them proper health education every clinic visit.
Upon the approval and direction of Consultant/Nephrologist, donors and recipients are prepared thoroughly for a safe and successful transplantation. Helps in the assessment and evaluation of a potential donor/recipient candidate. Fills up checklist information for potential candidates for transplantation. Secures bloodwork investigations, radiology, cardiology work-up and consultations to different departments. Ensures kidney recipients are prepared hemodynamically, emotionally and psychologically. Provides pre-operative nursing instructions, primarily against infection post-operative and compliance to medications. Hand-outs are provided as part of patient's education. Carries out post transplant investigations for in-patient and out-patient cases.
Hemodialysis Unit

Initial assessment to dialysis patients is done by checking vital signs, pre-dialysis weight and mental condition. Prepares dialysis machine making sure proper dialysis bath is used and dialyzer is intact. Positions patient properly on dialysis chair/bed. Checks patients vascular access, be it A/V fistula or perm. catheters for signs of infection. Makes sure that vascular access is patent and working well. Dialysis procedure is done properly making sure patient's condition is ok by checking vital signs regularly and no complaints has been made. Makes sure patient's dry weight is achieved post dialysis. Refers to attending physician for patient's unusual condition and complaints. Proper health instruction is done.
