Hospital is Accredited by the NABH and Department is maintaining as per the NABH accreditation Standard and to establish, organize, and manage a medical record department with appropriate systems to provide an effective service in the hospital, develop polices and procedures relating to the medical record department in accordance with the health Directorate/Ministry of Health, assist the Medical Record Committee in the design and development of different forms required for hospital use, to review the medical records of outpatients, Inpatients, and Emergency patients to ensure that they include all important documents and pertinent information, to cooperate with the medical, nursing and other staff persons in completing patient medical records, participate and assist in quality assurance, utilization review, infection control, and other committees and programs, to prepare and maintain medical reports, medical certificates, and death registers, and notify concerned authorities in duly completing the required procedure, to prepare monthly statistical reports concerning the hospital activities carried out, and to submit to concerned authorities any suggestions for improvements, to observe professional ethics and to protect the confidentiality of information from unauthorized persons; to keep medico legal records under safe custody and to attend court whenever required, to participate and assist in research programs to develop new methods and procedures for improving administrative activities and financial control, select appropriate personnel for the medical record department and train them for performing their jobs efficiently, and carry out any other duties and functions related to medical record services, etc.
Complete Re-Organize of 350 Bedded multi speciality Hospital's Medical records department with appropriate system to run the department with smooth function. Training given for all the staff members for the smooth function of the department for inpatient area, out patient area, Casuality, Injpatients records process, out-patients records process, orientation of ICD-10, Statistics, and census etc.
Working as a Medical Records specialist and Assisting to the higher authorities for the smooth functioning of the department. Handling the hospital statistics and collecting the data from all the department. Giving proper instructions to the Patients and subordinates about the admission And discharge formalities., Review all the inpatient discharged Medical Records Thoroughly assemble, deficiency check And assign accurate ICD codes in the patients Records. Indexing diagnostic and procedure and maintaining them Numerically in systematic manner. Giving Training in the ICD Code for the staff. Maintain the patient master index system. Giving suggestions to the management for the better medical Records, and active role In the hospital internal and external Disaster Plan, Design the hospital forms, etc.
