Case Manager, Psychologist
- Experience in NGO/INGO in IFH for 4 years
- Provide individual sessions for beneficiaries
- Create and provide group sessions, under the supervision of the Project Coordinator
- Work closely with the Outreach Team Leader in order to provide the necessary social services needed for the beneficiaries.
- Create treatment plans for each client on the caseload.
- Assist with creation and planning of activities for the beneficiaries
- Monitor on-going care plan development with special attention to safety issues and stabilizing the family from the effects of domestic violence.
- Establish long-term and short-termgoals in case planning, which can include family stability.
- Coordinate the provision of social services and to cases Mental health, families, and significant others to enable these to deal with the impact of Psychological disorders on individual family functioning and to achieve maximum benefits from healthcare services and Mental health
-Ensure involvement of local authorities, community leaders, and community groups in creating community network which increase access to better health and Psychological and mental health services and social services for vulnerable population
-Treat survivors in gender-based violence (GBV, GBVS), mental health/psychosocial disorders and other vulnerable(GBV, GBVS), beneficiaries with respect, kindness and in a caring manner, always maintaining confidentiality and based on survivors’ needs and wishes.
- Give survivors (GBV, GBVS), information about consequences of sexual assault, mandatory reporting regulations and help them identify their needs and options.
- Compile GBVIMS data on a monthly basis.
- Work closely upon consent of the survivor (GBV, GBVS), with the clinic and cash staff as well as other referral partners on referrals to ensure holistic care of survivors(GBV, GBVS)
- Participate in case conferences, file reviews and any other meetings as required.
- Conduct home visits and mobile service provision in community safe spaces, as needed.
-Identify individual cases through regular presence in the community and activate the referral role to provide another type of services through external agencies.
- Conduct initial (rapid) assessments for individual cases and prioritize them according to risk level.
-Regularly follow up to ensure all services and action points listed in the case plan are carried out within agreed time frames.
- Ensure that progress is regularly reviewed. Regularly monitor and support individuals of concerns and their families through home visits, providing guidance, advice and emotional support, community mediation and referrals.
- Work with supervisors and managers to arrange case meetings for complex cases and ensure assessments for individual cases and receive multidisciplinary support.
- Manage cases in line with existing SOPs, adhere to standard documentation processes and follow best practice guidance.
- Regularly document cases using case notes and other agreed upon forms, update databases to ensure a comprehensive record of the case.
-Provide one-on-one emotional support counselling and guidance to GBV survivors and other vulnerable groups
- Work closely with other WPE staff to sensitize community members to GBV and services available within the women’s centres.
- Contribute to the analysis of GBV caseload reported to the clinic, in order to provide feedback on needs and gaps to community workers and the wider WPE team
- Work closely with the Community Workers to provide outreach to encourage survivors to come forward and seek assistance.
- Compile and prepare weekly reports and any other data as required by the Senior officers, Deputy Program Manager and WPE Manager.
- Act as ‘Focal Point’ volunteer, Doctor, nurse, Youth Guide, Psychological guidance, Special Education Specialist, Hearing and Speech Specialist, Physiotherapist and Coordination with the team leader and senior management
My job is a psychologist and a case manager of Child Protection as Case Manager is to promote quality and effective outcomes through communication and the provision of appropriate resources to meet an individual child’s needs. The process includes comprehensive assessment, planning, implementation, coordination, monitoring and overall evaluation of individual children’s needs.
- Apply guiding principles in working with children, being guided by the best interest of the child in all actions at all times.
- Familiarity with child development and children’s wellbeing
- Communicate and work effectively and appropriately with children of various ages and their families, including those who have experienced very difficult situations
- Assess risks, needs and strengths and develop the appropriate response in consultation with the child and/or his caregiver(s)
- Collaborate with clients to plan, implement, monitor, and amend individualized services that promote clients’ strengths, advance clients’ wellbeing, and help clients achieve their goals. and make plans and shall have specific attainable, measurable objectives.
- Work with children, families and carers to implement the actions determined through planning to bring about the changes necessary to --ensure the safety, stability and development of the child. This includes directly guiding, encouraging and supporting the child and family while monitoring impacts and outcomes and Conduct safety planning with clients, working in collaboration with the client’s family (if/when appropriate) and other agencies to manage the identified risks
- Be the contact point for needs assessment and follow up
- participate in case conferences and family meetings with other professionals
- Document all case management activities in the appropriate client record in a timely manner, using the IMS as a database and case management tool
- Advocate for the rights, decisions, strengths, and needs of children and families and promote their access to resources, supports, and services
- Promote collaboration among colleagues and organizations to enhance service delivery and facilitate client goal attainment.
- Ensure involvement of local authorities, community leaders, and community groups in creating community based protection mechanisms
- Provide regular updates and reports to senior management staff as required; contribute to donor reports and internal clinic (IFH)reports as required
