changes in patient and pharmacy records and informs others of medication order changes
- Identifies drug therapy problems arising from therapeutic duplication, drug-disease
contraindication, drug-drug interactions, incorrect drug dosage or duration of drug treatment and
drug allergy interactions; reports observations to the physician and provides corresponding
recommendations
- Participates in the prevention, detection, and monitoring of adverse drug reactions and medication
errors; documents observations/findings and prepares corresponding report for submission to
