1) carrying on Bad Actor (BA) process from the BA items starting from identification, registration then priority ranking based on RAM scoring. Following that, generate corrective actions (recommendations) till acheive the desired resolution.
2) carrying on Mitigation Threat to availablity (MTA) process from the MTA items starting from Threat Identification workshop (TIW), registration then priority ranking based on RAM scoring. Following that, generate corrective actions (recommendations) end up with both mitigation and resolution plans.
3) conduct Formal RCA as Team Leader for All Rotating Equipment incidents ( refinery wise) following Apollo methodology & write RCA report that include full technical analysis, all failure facts, findings as well as the Reality Charting (RCA tree) which contains the root cause(s). also, generate the desired recommendations that will prevent the failure re-ocuurance.
4) generate and distribute weekly report of RCA, MTA&BA Actions items status.
5) deliver a presentation of RCA&MTA&BA actions status and Technical presentation as lesson from incidents (LFI) in monthly basis during Monthly Top Manegment Safety meeting.
6)assigned as Night Shift coordinator during Mega Turnaround 2019 at Cracking Plant.
8) participate as team member in all Informal investigation (using 5 why’s) for any system failure in Distillation plant as Lead Defect Elimination Engineer.
7) deliver Distillation Plant KPI presentation in monthly basis.
8) stand for Defect Elimination Department Manger as needed.
9) assigned for several management tasks.
