ROLES AND RESPONSIBILITIES:
* To ensure that every worker is met with Prerequisite like PTW, ID Proof & PPE
before allowing entrant to worksite.
* To maintain Permit Renewal Sheet, Gas Test Record Sheet, Ventilation and Rescue
Plan
* To maintain up-to-date NLTPHRW Sheet.
* To maintain Grinder Check List & Post Welding Heat Treatment Check List
* To maintain Log In / Log Out Sheet.
* Communicate with RADIO PANEL OPERATOR to confirm the Start and end of the
work.
* Maintain frequent communication with the worksite entrant.
* Barricade the worksite entry Point when not in use to prevent unauthorized entry.
* Monitor continuously the condition inside of worksite and look after any sign of
smoke or fire.
IN CASE OF EMERGENCY IN CONFINED SPACE:
* Order worksite Entrants to evacuate immediately.
* Report emergency situation via Radio or IS Phone.
* Use the Fire extinguisher from outside of worksite but only if all personnel already
evacuated worksite
