- Perform daily medication chart review and monitor the therapy for contraindications, drug-drug interactions, drug-food interactions, allergies, appropriateness of drugs and the dosage.
- Evaluate drug therapy orders for appropriateness of dosage, route, frequency, schedule, and duration throughout patient’s hospital treatment course. Intervening by direct communication with the physician when needed.
- Suggest appropriate therapeutic alternatives for Non-Formulary Medications.
- Prepare and dispense medications including oral preparations, sterile preparations, TPNs, and chemotherapy safely and accurately using appropriate techniques.
- Adjustment of the dosage regimen based on TDM results and the patient’s clinical response.
- Document all clinical activities and interventions accurately and completely.
- Responds with the code blue team in case of CPR when called.
- Responds to the hospital poisoning hot-line.
- Dispensing outpatient prescriptions and prescriptions for discharged patients.
- Dispensing inpatient orders based on unit dose dispensing system.
- Preparing and quality-checking sterile medications including IV medications, TPNs and IV Fluids.
- Providing patient education for outpatients.

- Counseling patients about proper use of medications either prescription or OTC medications.
- Maintain records, such as pharmacy files, inventories, registries of poisons, narcotics, and controlled drugs.

Responsibilities:
• Reviewing patients’ medication regimens regularly.
• Rounding with medical teams to provide drug information and collaboratively monitor patients’ responses to their medications.
• Adjusting medication doses based on KFT, LFT and Age.
• Screening patients’ medications for Drug/Drug and Drug/Food Interactions.
• Providing patients medication counseling for in-patients or upon discharge.
