o Responsible from all medical reports in the hospital.
0 Classifies and codes diseases both topographically and etiologically.
o Reviews admissions, treatment and discharge records for completeness.
o Compile monthly movement reports and special reports related to Medical Records Department.
o Review records of medical services rendered to determine the accuracy of charges submitted by doctors and hospitals.
o Reviews medical record and reports by medical stenographers.
o Develop and implements complex medical records system.
o Responsible for the assembly, the qualitative and quantitative analysis of all patient’s medical records and the filing of patient records and loose reports.
o Under the supervision of the Head of Medical Record, the Medical Record Officer is responsible for the assembly, the qualitative and quantitative analysis of all patients' medical records, and the filing of patient records and loose reports.
o To perform checking and procedure requirements for all outpatient and discharge, patient medical records submitted/retrieved to the department.
o To organize and standardize the filing order of the outpatient and discharge patient record.
o Analyzes 10 discharge inpatient records per hour and day cases with 99% accuracy.
o To manage and work alone in a Satellite Clinic.
o Follows policies and procedures to contribute to the efficiency of the Coding/Analysis area in particular and the medical records department in general.
o Assist in the file room whenever needed. Separate inactive, terminated charts, check VIP list and the unreturned files.
o Using terminal digit filing system, files completed discharge summaries/operative reports in their appropriate location. Ability to file 75 loose reports per hour.
o Monitor office supplies and equipment’s, and make material requests.
o Trace missing and unreturned files.
o Works as a team leader in all projects within the department and in all shifts of the filing room and other sections of the Medical Records Department.
o Identify, compile, abstract and code patient data, using standard classification systems.
o Prepares and completes deficiency sheet and place of medi-tag on each deficiency notes. Prepares physician and nursing staff notification of chart deficiencies and assist them with completion of deficient records.
o To organize and standardize the filling order of the outpatient and discharge patient record.
o Manage the archive unit and purge of the inactive files.
o Makes appropriate follow up of all missing records. If record is not available after 24 hours, notify the manager of Medical Records to assist in locating the chart.

o Assembles discharged inpatient records according to specified chart order with 99% accuracy.
o Assist in the final reviews of completed records, updating the chart completion system appropriate.
o Collects and compile data from Medical Records for statistical and other related reports.
o Make volumes of medical records and merge duplicate files.
o Ability to analyze in patient files.
