SENIOR CLINICAL PHARMACIST
à OMEGA HOSPITALS
Lieu :
Inde - Hyderabad
•Provide comprehensive clinical pharmacy services & promote rational medication use & monitoring of drug delivery systems.
•Provide, educate, & advise the evidence based practice to all health care staff on optimal drug selection.
•Participate as an integral quality member over matters arising for medications in the hospital based PTC, Infection control, Medical audit & CPR committee's.
•Lead, Train, Monitor, Educate & Co-ordinate with 3 clinical pharmacists in 3 other Omega Hospital based satellite centers.
•Medical Audit for comprehensive medication management, ADRs, Medication Errors & Anti-Microbial Stewardship.
IN PATIENT SERVICES
•Prescription audit of in patient active case sheets.
•Identify oncology prescriptions that may present risks to patients & resolve issues during dilutions of IV admixtures.
•Discern drug errors by assessing potential risks in prescriptions & strategies to mitigate them with interventions.
•Participate in consultant rounds & disseminate drug information.
•Collect NABH based MOM Indicators, a tracking method improve quality & service outcomes.
•Audit & regularization of Narcotics in the hospital.
•Compilation of medication reconciliation, allergies & past self-medications in the HIS.
•Check for drug appropriateness in HIS - Identify errors - Notify the clinician for further modification.
•Identify, document & report all ADRs arising in hospital.
•Monitor & supervise drug delivery systems & chemotherapy IV admixtures during administration.
•Cross verify the discharge summary information for drugs followed by patient medication counseling.
•Death case sheet audit as per quality standards at regular intervals.
•Audit of medications in crash cart and bio medical waste.
OP SERVICES
•OP prescription audit.
•Medication Counseling to patient/patient attendees.
REPORTS
•Prepare & compile audit reports into monthly reports & share them with the respective staff in monthly scheduled committees & also with the hospital medical administration.
