COORDINATION WORK JOB SUMMARY
Perform patient assessment using appropriate problem focused assessment techniques.
Participate in multidisciplinary meetings.
Initiate appropriate discharge plan.
Document patient and family education.
Apply safety measures related to patient care.
Act as a liaison between patients, referring doctors and the BMT team and perform all
processes required from referral to admission of a patient for BMT treatment
Perform all pre-transplant work up and scheduling required for admitting patients
Arranging patient visits and physician consults,
Scheduling pre transplant screening and required evaluations to determine patient
eligibility for BMT
Coordinate patient and family education
Monitor patient/family before, during and after the transplant process for physical,
emotional and/or psychosocial needs and intervenes when appropriate
Develop and implement teaching plans for patients concerning promotion, maintenance
and restoration of health
Updating BMT database and statistical analysis
Annual reporting of new transplant patients
