Authorized Gas Tester:
Visit spot (confined space) required gas testing location.
Review new and existing permits for hazard identification.
Review confined space entry and its related activity documents (MS, TRA, Checklists).
Make sure the availability of all required precautions for confined space entry prior gas testing. (Safe access and aggress, appropriate Lifeline, Proper ventilation, adequate low voltage lighting, signs, competent stand by person etc.)
Check tool box talk (TBT) has been conduct with all entrants.
Review all entrants are confined space entry certified and certificates are valid till date.
Make sure all entrants are aware of emergency escaping plan.
Carry out gas testing with appropriate equipment’s and due calibrated
Prior sign the permit make sure it’s safe to work as per standard level of H2S, Hydro Carbons and Oxygen etc.
Keep monitoring and carry out gas testing as per location and activity requirement (Continues gas testing, after 1 hour work break gas testing, daily gas testing)
Permit to Work Coordinator:
Overall responsibility to the Site Controller for the safe control of work activities; this includes issue of work control.
Coordinates and participates in work permit co-ordination meetings.
Audits the Permit to Work Procedure and ensure compliance on a daily basis.
Ensuring all the necessary signatures have been obtained on the permit and informing the JP, IA and AA of any potential conflicts.
Operating the Permit Office to reflect the current status of all permitted work
Taking a lead role in ensuring the SIMOPs is not compromised by the issue of conflicting permits
Takes part and contributes meaningfully during PTW review meetings.
Forward all audit reports to Manager and records to be maintained.
Ensure the appropriate level of risk assessment has been carried out for the task and that this has used the hierarchy of control measures. Consultation of the risk Matrix Table (RMT).
Ensure that the Job Performer carries out a pre-job tool box talk at the work site with the other members of the work party before any work commences.
Demonstrate detailed knowledge of Golden Rules of Safety and Control of Work related procedures
Keeps track of valid work permits and ensures closure of work permits on completion of work. Ensures expired work permits are closed, cancelled or renewed as the case maybe.
Assessing and reporting of unsafe work and where appropriate reporting these to the site controller.
Give Internal HSE Training & Safety Induction to the new joiners and Employees.
Check MSDS (Material Safety Data Sheet) of Gases and Chemicals.
Visiting sites for monitoring and checking safety issues prepare near misses/incident & accident reports, ,
Carry site inspections, vehicle inspection and equipment’s; Welding Machines, Grinder Machines & Drill Machine, Air Compressor Inspections
Check fire extinguishers, smock detectors, fire hydrant cabinet’s, fire water and fire pump & fire alarm system and conduct trainings and drills.
Implement HSE Rules at sites as per ADCO HSEMS.
Check the lifting gears and equipments with correct color code and valid testing certificates
Implement HSE polices according to company procedure.
Ensure tool box talk conducted at site prior commencing any task.
Check wind pressure for lifting equipments with cranes.
Implement ROAD SAFETY POLICIES as per ADCO Safe Driving Procedures.
Conducting weekly or monthly trainings with drivers and discuss night driving hazards, fog driving, seat belts safety, mobile phone prohibition during driving, no drunk drive, No Smoking while drive etc.
Conduct BBSA (Behavior Base Safety Audit) for Drivers & Workers
Reporting to the management by finding of unsafe acts.
Enforcement on workers to implement Safe System of Work at site.
