Sick Leave Reporting
Receive all the sick calls
Make sure that the staff called the immediate Manager (during working hours)
 Notify the ward (when applicable) and adjust staff if required
 When applicable (as per the Sick leave required list) the Admin Supervisor will ask for the Sick leave note.
 Sign on the sick leave
 Original sick certificate (for HR) through Nursing Admin
Copy to the staff-member’s manager.
Administrator on call
 Death & still birth notification
 White copy to medical records, green to admin supervisor, yellow to family
 Name on the notification to be the person who will go to police in person along with own ID and the patient’s ID
 Complete the number of death list & send through email to pharmacy director, PI, admin supervisors, medical records, ACNO, CNO, executive assistant to CNO
OR Team
 On-call list - location
 Call Surgical resident medical on call (RMO) when required
 Call the 1st OR on-call team for any emergency cases.
 Call the 2nd OR on-call team if the 1st OR team busy with an emergency case.
 Fill in the emergency surgery booking form.
 Check the OR case during surgery and PACU

a. Go to the code
b. Act as recorder and time manager
c. Responsible for monitoring quality of code: CPR, Ventilation, rotation, equipment
d. Secure signature of the MRP on the record
e. May use the application on the pad/phone and then transcribe
f. Fill in the CQI on the QM: assign manager of Life support and PI
g. Send the original record to the medical record supervisor for uploading on Meditech
h. Notify MRP if not present or already notified
i. Notify family members

3- After the code:
a. If successful: clear the code, document and reassure family.
b. If unsuccessful, do all the above and secure death certificate, document the details in the death notification book,
c. Explain post-death process to the family.
d. Transfer patient to appropriate place: ICU, Mortuary or release the body…
