Clinical resourse for RN's, dietician, social worker and technicians who provide care to End Stage Renal Disease patients. Supervises a team of RN's and tecnicians. Reports to Medical Director. Analyzes patients assessment, laboratory data and reports results to MD. Educates and trains staff. Implements new initiatives and enforces company policy and procedure. Assess patient and family learning readiness and provides information regarding disease process and care. Ensures that documentation is complete. Ensures that there is adequate staff and supply. Ensures that equipments are properly maintained.
Provides hemodialysis to End Stage Renal Disease patients. Reviews laboratory results and makes changes in the Epogen and Zemplar dosing per protocol. Ensures that patient's receive correct doses of medication to prevent anemia and to maintain normal hemoglobin values. Reports to physician PTH levels that are unmanageable by Vitamin D doses. Ensures that crash cart is complete and well-stocked, medication are not expired and equipment are working properly. Reports broken equipment for service and prompt repair. Prepares report on vascular access status of patients. Ensures that records are complete and kept confidential. Assists physician during rounds, consultation and examination. Trains newly hired nurses and technicians.
Provide professional nursing care to clients with due respect of their race, gender, beliefs and confidentiality. Perform thorough physical assessment including past and present medical history and family history. Prepares and administer medication as ordered. Ensures that medical supply is complete and well-stocked. Performs venipucture to obtain specimen for laboratory examination as ordered. Reviews laboratory results and reports values that are out of range to physician.Capable of providing nursing procedures performed in a clinic or school setting. Competent in using ECG, spirometry, audiometry, taking and monitoring vital signs and using common office equipments like computers and fax machines. Able to perform quick, careful assessment in emergencies and decide which patients need immediate attention, who needs to be transported promptly and who can wait without compromising their medical condition. Provide immediate life support treatment to patients who are hypoglycemic, having cardiac emergencies like atrial fibrillation, supraventricular tachycardia or ventricular tachycardia or cardiac arrest. Ensures that medical/nursing records and flowsheets are accurate and completed in timely manner. Assist the physician during rounds, consultation and examination and provide report on patient's clinical progress and treatment. Refer patients to other medical specialty as ordered and sets up appointment and their transportation. Attends compliance and quality assurance meetings to address issues in the clinic. Attends patient conference together with physician, dietician and social worker to discuss treatment progress and draw care plan for specific patient need. Keeps abreast with changes in company policy. Ensures that clinic is safe for patients and staff - wet and flooded floors are cleaned up to prevent slip and falls, defective machines are reported for repair promptly. Maintains an environment where patients feel safe and welcome and can communicate their need

• Coordinates team effort to deliver top quality patient care and to meet goals set by the company such as DQI, anemia management, bone and mineral management, dialysis adequacy, catheter reduction, vaccination and waste reduction in company resources

ANEMIA AND ADEQUACY MANAGER
• Reviews laboratory results and recommends adjustment to Epogen and Iron doses per protocol
• Implement anemia management protocol
• Documents appropriate medical necessity for initial and continued erythropoietin therapy
• Train, coach and mentor new nurses in anemia management
• Reviews laboratory works and makes recommendation to MD to improve dialysis adequacy
• Communicates with patient and family any changes in the plan of care
• Ensures that all approaches are utilized to meet KT/V of 1.4 on all patients
• Collaborates with the team to meet adequacy goal of every patient
