• Make rounds in the different areas/units assigned. Update patient profile.
• Make medication checklist and collect data for the profile.
• Check all medication order of each patient assigned for completeness/appropriateness.
• Review and evaluate the new order (s) including the immediate and discontinued.
• Conforms medication order on the automatic stop policy of the institutions.
• Prepare all standing medication order that is in the formulary system.
• Endorse all prepared medication to nurse on duty in charge of medication for proper storage and right frequency to administer.
• Inform the prescriber of any untoward effects and delayed reaction of the administered medication on the assigned patient.
• Follow up the adverse drug report (ADR) and make documentation.
• Routinely check the e-cart (emergency cart) every 15 days and replenishes after each use.
• Dispense ethical regulated and controlled substances if it conforms within the policy of the institution.
• Maintain the drug delivery system and the unit dose system orderly and correctly.
• Availability of the medication is check from the time it is ordered and the proper storage.
