• Supervise and monitor the daily work and achievements of CHWs and PHCCs' focal points & Assist in preparation and monitoring processes.
• Collect, organize, and archive all documents and Means of Verification related to distribution and monitoring activities.
• Build CHWs' capacity in health promotion topics, including Child Nutrition, Covid 19 prevention, Breastfeeding, ANC & PNC, IYCF, immunization, PHCC services, malnutrition screening, Early Childhood Development and others.
• Support PHCCs' focal points in monitoring CHWs' outreach activities, collecting field databases using MERA and Phenics Software, setting targets, and handling referrals using RIMS & Inter Agency Referral form.
• Monitor distribution activities and related documentation, providing support to PHCCs' focal points and CHWs.
• Regularly reporting on progress, achievements, and challenges, as required
• Conducted candidate selection and interviews to identify qualified CHWs for project engagement.
• Coordinated with Health Facility Directors and municipalities to secure authorization for CHWs' health awareness sessions and household visits.
• Supervised CHWs, overseeing daily activities, collecting weekly reports, and maintaining awareness data in Excel.
• Tracked and monitored CHWs' accomplishments and targets, ensuring alignment with project goals.
• Established a communication link between CHWs and supported health facilities to ensure vulnerable families received appropriate care.
• Built CHWs' capacity for communicable disease control, identification and referral of GBV survivors, mental health issues, CP and malnutrition.
• Raised awareness among CHWs and communities on health promotion for early outbreak preparedness and disease prevention.

• Monitored stock of distributed items (fabric masks, micronutrients, Oral Rehydration Solutions) using proper tools.
• Referred suspected cases of measles, cholera, hepatitis A, and others to the MOPH for rapid response using DHIS2.
