Duties and Responsibilities:
o Monitor patient’s condition through proper assessment and vital signs taking.
o Formulate plan of care based on assessments, implements care and evaluate them every shift.
o Assist the patients in their activity of daily living, including maintenance of hygiene, bed baths, daily changing of wound dressings, skin tractions, etc.
o Assist the patients in diagnostic/laboratory examinations and in preparation on surgical procedures.
o Assist and monitors patient’s condition pre and post operatively.
o Accompany and assist doctors in their daily ward rounds, carried out their orders accordingly.
o Administers medications accordingly, (guided by 6 Rights of giving medications: right patient, right drug, right route, right dose, right time, right documentation).
o Provide health teachings to promote care and attended seminars/trainings to upgrade knowledge and skills in providing care to patients.
o Make proper documentation in each patient’s condition.
o Ensures patient safety all the time.
o Ensures patient confidentiality and privacy in related to one’s health condition.
o Prevents nosocomial infection to spread from one patient to another.
o Be patients’ advocator and recognize the patient’s rights.
