•Review medical records and determine if they are complete and accurate.
•Ascertain that medical records are in support of individual risk adjustment score accuracy.
•Develop plans and materials to support education and system changes, to meet both practice and revenue goals.
•Ensure knowledge on deductibles, co-payments, co-insurance amounts, insurance exclusions and other policies of all insurances that hospital is dealing with.
•Educating physicians in Improving Outpatient documentation for reaching correct diagnosis & procedure codes for better reimbursement.
•Review medication list to verify if there is a correlating condition and if conditions are still being treated.
•Review all specialist and hospital consult and lab and pathology reports for new and more specific conditions.
