Unite Description: Primary Health Care Center - 24 Hours ER Service, Al-Iskan Taif, PHC, Western Region.

Al Iskan Taif clinic is a primary health care center, located within the National Guard housing compound, which serves a population 50, 000 and monthly patient volume approximately 15, 000. The scope of care includes a 24 hours emergency service, care of both acute and chronic conditions, health education, prevention and control of endemic diseases, maternal and child health care, preventive and promotive medicine for clients of all ages.

Principle Duties:

1. Provide direct patient care utilization the nursing process within established hospital, divisional and unit specific policies, standards protocol.
2. Take Direct and appropriate action in urgent and emergency situations.
3. Documents all assessment, interventions and other patient information in the patient's health care record in a timely and ongoing manner.
4. Promotes continuity of patient care through oral and written communication.
5. Demonstrate effective communication and collaborate skills when interacting with patients / families other health care team members and staff of other departments.
6. Acts as unite management recourses e.g., care coordinator, charge nurse and team leader.

Clinic Practice:
1. Assess and triaging patients based on complaints and problems.
2. Carrying out of doctor's order according to their treatment such as giving nebulization, administration of drugs, I.V fluids, doing ECG and others.
3. Escort patients via ambulance to the other hospital.
4. Respond to a house call according to the policy of NGHA.
5. Recognize and takes appropriate action in emergency situations and using priority setting skills.
6. Report relevant clinical progress to medical staff and nurse manager.
7. Evaluate and documents effectiveness of nursing care.

Duties and Responsibilities:
1. Provides education related to diabetes and the disease process; medication, diet and nutritional monitoring; foot care, prevention and care of complications; blood glucose monitoring and the techniques of self-monitoring and injection.
2. Assesses patient educational needs by identifying physical, social and or emotional needs that may influence the education process and the long-term outcomes.
