The main part of my current position is the direct clinical care on AMU (Acute Medical Unit) in form of daily ward rounds. We start daily with the morning handover meetings after finishing the post-take round by the consultant on-call. Around midday there is a ward MDT at 12:30 to discuss problems, allocate patient and discuss potential discharges. As consultant body of the acute medicine service we offer daily post-take round reviews from 13:00 to 17:00.

To improve the care of patients with respiratory disorders on AMU I review of patients with respiratory syndromes and perform specialty related procedures including thoracic ultrasound and related interventions.

The X-ray meeting on Tuesday 12:30 is part of my job description, where we discuss complicated respiratory cases and suspected cancers.

I also cover a Tuberculosis clinic on Wednesday between 13:30 to 17:00. In addition and as part of the tuberculosis services I regularly at ten the TB multidisciplinary team meeting.

Frequently I participate in the postgraduate teaching programme for house officers on Tuesday & Friday. I aim to cover a wide range of respiratory topics that are relevant for both acute and general medicine such as asthma, COPD, pneumonia, pneumothorax, pleural diseases and lung cancer.



Currently I have developed a full plan for a medical HDU including staffing, equipments and admission algorithms & guidelines. We are awaiting the decision of the executive board.

I am also working together with with my consultant colleagues from the infectious diseases department to develop a pathway for pleural investigations.

Also I was asked to be the mortality lead for Acute medicine in the hospital. We are holding regular internal meetings to discuss cases and I´ll be presenting our data soon in a trust wide M&M meeting.

Unselected acute general medical on calls as well as weekend discharge rounds with junior doctors are an important part of my job.

My work spectrum included covering community clinics eg obstructive airway disease and general respiratory clinics. Community clinics are very holistic and aim to support patients with treatment plans, physiotherapy, smoking cessation, inhaler technique and self monitoring of symptoms. The primary focus is to educate patients in understanding their illness better.

Additionally & according to the rotation plan I also covered the respiratory ward at Watford General Hospital and supervised junior doctors. It was a general respiratory ward where we treat and diagnose a wide spectrum of disorders including, COPD, lung cancer, bronchiectasis, asthma and ILD (interstitial lung diseases) and respiratory infections including tuberculosis. The ward also manages unwell patients with NIV and tracheostomies.

Part of the job was reviewing recently admitted patients in the Acute Admission Unit (AAU) or Ambulatory Care Unit (ACU). Responding to internal referrals from other wards including those of the Intensive Care Unit (ICU) is covered by both Consultants and the SPR`s.

I performed a wide Spectrum of diagnostic procedures and investigations, including flexible bronchoscopies with broncheoalveolar lavage and biopsies, thoracic ultrasound, pleural fluid aspiration & intercostal chest drain insertions.

In this centre we are actively involved in the Diagnosis and Therapy of interstitial Lung diseases, Lung cancer, Pleural diseases, Tuberculosis, COPD and Asthma. I was running three outpatient clinics for general respiratory medicine. I developed expertise in outpatient evaluation and management of pulmonary diseases.

Supervision, training of junior staff and the Improvement of the Training Programme were part of my Job. I offered the respiratory staff (including doctors, respiratory specialist and ward nurses) a weekly teaching session on Friday covering a wide range of respiratory subjects.

The respiratory ward in Ealing has 40 inpatients including those needing NIV. We had frequently several patients on NIV simultaneously requiring intense care, supervision and input. Patients with a wide range of disorders were present on the ward including pneumonia, asthma, copd, bronchiectasis, respiratory failure, interstitial lung diseases, lung cancer & pulmonary tuberculosis. The ward MDT took place on Monday 11:30am aiming reducing length of stay, optimising care, discussing problems and planning care for severely sick patients.

I was actively involved in organising and conducting clinical audits (pneumonia, COPD, chest drain management).

I attended the weekly lung multidisciplinary team meeting (MDT) and was actively involved in the discussions and making decisions regarding patient and case management.

I also attended once monthly the tuberculosis multidisciplinary team meeting.

I participated in unselected acute general medical on calls as well as weekend discharge rounds with junior doctors.

Academic teaching hospital of the Westfälische Wilhelms University of Münster, Germany. The respiratory Department covered a wide Spectrum of Diagnosis and Therapy of interstitial Lung diseases, Pulmonary Hypertension, Lung cancer, Pleural diseases, Tuberculosis, COPD and Asthma.
I also gained experience in the pre-operative evaluation of high risk patients, including cardiopulmonary exercise testing and regional ventilation perfusion lung scanning.

I was continuously responsible to supervise junior doctors and run a part of the respiratory ward, be involved in discussions with other department, speak to patients and their families and organise the ward work.


We offered and performed daily a variety of therapeutic and diagnostic procedures like rigid and flexible bronchoscopy, EBUS (endobronchial ultrasound), tumour debulking (APC, Foreceps and cryocoagulation), endobronchial stent, lung volume reduction with valves and coils implantation, local and transbronchial biopsies, bronchoalveolar lavage, pleural aspiration and drainage, pleural biopsy.

During my time there we established a high dependency unit called " Respiratory Intensive Care Unit", where we managed mainly patients after extubation or tracheostomy as well as patients with acute respiratory failures with NIV management.

An important part of the job was the management of patient with severe respiratory complications in the intensive care unit which was our own responsibility. We provided complete management starting with intubation, treatment plan, performed all necessary procedures, extubation and weaning, made all strategic decisions and presented the long term plan.
