I. CLIENT CARE
A. Sets priorities in performing immediate nursing care to: • Acute-ill clients - 30mins to 1 hour
• Moderately-ill clients - 30mins to 1 hour
• Ambulatory clients - 30mins to 45mins

B. Attends promptly to the immediate basic needs of acute-ill patients such as: • Gives resuscitative measures to clients undergoing cardiac arrest.
• Accurate measurement of client's intake and output of those suspected of acidosis or undergoing shock, those with which evidence of massive bleeding and sudden loss of consciousness.
• Test's client's level of consciousness through therapeutic communication technique and procedures in order to know client's mental capacity.
• Ensure client's safety to avoid added injury.

C. Attends to the basic needs of moderately-ill and ambulatory client's.
• Takes client's vital signs and takes note of any deviation from the normal values.
• Symptomatic relief of client's discomforts like catheterization of clients with distended bladder and difficulty or inability to urinate; rendering tepid sponge for slight temperature elevation; oral hygiene and ice chips for those with persistent vomiting; administering oxygen inhalation; Psychological support for clients suffering from any form of depression; giving medications to relieve clients discomfort upon doctor's order; analyzes carefully and arises out promptly physician's prescribed treatments; assisting physician in performing special procedure such as paracentesis, thoracentesis, suturing of lacerated wounds; plans and gives health teachings to the clients and family members towards promotion of health.


II. ADMINISTRATION AND SUPERVISION
• Assists in orientation of new employees and nursing students to help them familiarize with the set-up and activities.
• Participates actively on all staff and general meeting for the development of the area.
• Inspects and replaces the functions of the standard equipment and supplies before and after its use.
• Supervises ward aides, affiliates and janitorial personnel for effective client care.

III. RECORDS AND REPORTS
• Updates client's records.
• Accomplishes promptly and accurately the twenty four hour nursing reports after each shifts.

• Receives bedside endorsement from the outgoing staff; observe client's conditions, checks IVF, oxygen inhalation, indwelling catheters, thoracostomy site and tube patency, etc ; evaluates and observe clients needs and assesses which among them needs to be attended first (prioritize the needs)
• Insert IVF, NGT, and Indwelling Foley catheter as ordered by the doctor.
• Taking and recording vital signs, administers oxygen inhalation therapy; suctions secretions, notes the level of consciousness, catheterization of clients to relieve pain; administering of oral, intramuscular and intravenous medications under the supervision of a resident physician and legally with written order.
• Supervises ward nursing attendants, nursing students and nurse orienteer in bedside care management.
• Provides health teaching upon admission and prior to discharge.
• Properly document all what is done to the client and other circumstances that arises during the tour of duty.
• Assists medical residents in special procedure.
• Daily services with consultants, residents and carries out doctor's order.
• Practice of aseptic medical techniques at all times and terminal disinfection of client unit or bed with proper coordination with the housekeeping services.
• Measures accurately client's intake and output, especially cases with massive bleeding, sudden loss of consciousness, renal and cardiac cases and those suspected of acidosis or undergoing shock.
• Gives psychological support to clients suffering from any form of depression or suicidal cases. Proper placing of bedside rails or restraining the client in bed to avoid fall or avoid hurting her/himself.
• Provides emotional, spiritual care to critically ill or dying client.
• Coordinates with other member of the health team to meet the other needs of the client.
• Machine/Equipment handled: ECG machine, Suction machine, Defibrillator, BP apparatus.
