Responsibilities and Accountabilities
1. Assist S/MHP in supporting the Programme Clerk (Health Care) for health information and database administration of medical information both paper-based and electronic of medical record from any level of health care to be linked to medical profile and statistical output
2. Assist the Programme Clerk (Health Care) to collect health service utilization data form health service provider (hospital, clinic, laboratory etc.) to be input in the database of health information system
With assistance of Programme Assistant (Health Care), collect data from health activity such as health promotion, education, workshop etc. to be input into the database of health information system
3.Assist the Programme Assistant (Health Care) to collect medical record from health service provider and beneficiaries and transform it into electronic medical record and upload it into database of health information
4.Assist the Health Care team in monitoring and evaluation of health programme by conducting inspection, and data collection such as for hygiene and sanitation monitoring of accommodations including food and rations.
Compile, if necessary, and read the minute of the latest health staff meeting and the action points noted in the meetings.
5.Assist the Health Care team to ensure all medical documents, old and current are scanned and saved according to the program electronic filing system and hard copies are managed or kept in line with the requirements.
6.Assist the Health Care team in receiving and checking all incoming invoices from suppliers, services providers, and submit document for verify process by health team
Undertake any other related duties as may be assigned.

1. Material Warehouse (GMT)

Checking raw materials and packed material
Sampling of packed materials
Filling the receipt log book
Checker loading process
2. Production Department
Task and Responsibilities :
Calculate daily defects of product and make pareto chart
Measure and check temperature of pasteurized and pre cooling process
Stick transfer form from production to GFG
3. Finish Good Warehouse (GFG)
Task and Responsibilities :
Checker loading process
Inputs daily report Finish good warehouse
Fill daily loading plan board
Sorting product return
Notes product defect when loading process
Final Project : Improving Product Design of PackagingTea Drinks Using Quality Function Deployment (QFD) Method Summary : the problem in this final project is how to improvement a quality in order to design products considered by consumers using Quality Function Deployment (QFD) method, the goal is to improving quality with design products based on consumer voice which can be input or advices as a form of consideration to improving quality. Result of design with QFD obtained final design appropriate with what consumer want so can be shorten time to market and reduce the design cost.
Final Project : Use Statistical Process Control Tools For Quality Control Process Of Packaging A Product Summary : The problem in this final project is there are some mistakes in the production process causing packaging defects in production process, by using statistical tools can be measured how the level of product damage that can be accepted by a company with determining the tolerance limit of products produced. The statistical tools are used is P control chart and Pareto Chart. Using P control charts can be seen whether the process of quality control process by company is in control or not, and using pareto chart can be known the defects most take effect.
